
The Sage House Model works effectively because we do not duplicate what others are already doing within the community; instead, we work to partner with organisations to bring good quality services and care into the Sage House hub for customers to access in one place.
National Partners
We have forged many partnerships in the development of the Sage House Model and continue to expand the range of organisations we work with to bring our vision to communities all over the country.
Some of our partners include:

Sussex Partnership NHS Foundation Trust – We work closely with the Dementia Assessment Service, and they are co-located within Sage House Tangmere to provide diagnostic services. We also work with other NHS Trusts on co location in the replication of the Sage House Model.
The University of Chichester has undertaken an academic evaluation of the Sage House Model which lasted 18 months. We also work with them in partnership on other research projects.

We are working in partnership with the University of Oxford in the development of new AI technology aimed at improving support and information provided to carers of people living with dementia.
Sage House Tangmere has been chosen as a charity partner of the Goodwood Estate group; The Duke of Richmond is also a Patron of Dementia Support.

We partnered with Social Finance on a test and learn project to take an outcomes focussed approach to improving care for older people with dementia and other needs. We are now continuing our work together to explore ways of scaling interventions for people with dementia to improve quality of life outcomes and deliver savings to the local health and social care economy.
The Greenham Trust is partnering with us to support the replication of the Sage House Model in West Berkshire.
